Application Process

Please submit all application materials via email to tlara@conet.ucla.edu. Further details are given below.

• Partner information: Please provide all contact information for your community partner including contact person, phone number, email address, and mailing address.
• Statement: Attach a written statement of no more than three pages describing your partnership. Adobe Acrobat (PDF) format is strongly recommended; Microsoft Word and Rich Text Format (RTF) are also acceptable.
• CV/Resume: Attach your CV or resume.

In addition to the online materials described above, three reference letters are required:

• One from the executive director of the off campus community organization, or from a community leader familiar with your work and that of the community organization. This letter should include a list of the organization's board of directors.
• One from a colleague within the community organization or campus.
• One from a community member who has utilized the services of the community partner.

Letters should be mailed or brought to our office in hardcopy form, on the sender's letterhead, rather than electronically, and should arrive by the application deadline.

Applications for the 2008 Rosenfield Prize were to have been submitted beginning Monday, December 3, until Friday, January 11, 2008.
